Steps to Get Started
1. Complete Intake Request Form
Take a look at insurances we accept and if we are in network with your insurance. Explore the therapist's page and specialties to see if we would be a good fit, then complete an intake request form! Therapist will respond within 1-2 regarding if we are a good fit for services at this time and have openings. Please note that the therapist is not on in Fridays, so you will hear back early the following week. Please indicate on the form if your preferred method for response is email or phone call.
2. Complete Intake Paperwork
We utilize a client portal, through Simple Practice, to send paperwork needed to be signed and completed before an appointment can be scheduled. If we are a good fit, therapist will send via the portal. The intake paperwork must be completed within 48 hours to secure a spot, please check any spam or junk folders for link.
3. Who Signs the Paperwork?
- If the client is a child under 14, intake forms must be signed by all legal guardians of the child. You can read more about our policy and how it might apply to your specific situation, please reference the custody and consent page for more information.
- If the client is 14-17 years old, some forms will be signed by the teen and others by a parent or guardian.
- If the client is 18 or older, the client usually will sign all the paperwork unless discussed others.
4. First Appointment
For our office appointments, we do not have a receptionist, so when you arrive please make yourself comfortable in the waiting room. Your therapist will meet you there at your scheduled appointment time.
For telehealth appointments, you will be sent a video link to get started. Please ensure you are in a safe confidential space, without distractions, this can include a bedroom or office for example, but not a public location. Our video platform is secure and easy to use, but please call if there are any issues.
Whether you choose the office or online therapy, the first appointment will be spent getting to know you, as well as reviewing our the procedures for the practice. The therapist will ask more in-depth questions regarding treatment means and potentially assess symptoms being experienced, you will be asked to provide details of other services involved in treatment, and providing details of your presenting problem for wanting to start therapy services. If you have requested online therapy, your first appointment will include an assessment of whether telehealth is appropriate, we provide telehealth services to individuals over 14 if appropriate.
